眼瞼型特発性眼窩炎症（眼窩偽腫瘍）の臨床的特徴と治療効果

要約　目的：眼瞼型特発性眼窩炎症の臨床症状と治療効果の報告。対象と方法：過去4年間に診断した特発性眼窩炎症43例のうち眼瞼型18例を対象とし，臨床症状と治療効果を検討した。結果：男性12例，女性6例で，年齢は25～73歳，平均52歳であった。片側が17例，両側が1例であった。症状または徴候として，眼痛が67％，眼瞼腫脹が67％，視力低下が28％，眼球運動制限が39％，ぶどう膜炎が17％，眼内腫瘍に類似する所見が22％にあった。症状は高用量ステロイド投与で8例中4例（50％），低用量ステロイド投与で10例中6例（60％）が改善した。両群間に有意差はなかった（p＝0.35）。結論：眼瞼型特発性眼窩炎症では，ぶどう膜炎または眼内腫瘍類似の症状が起こることがある。症状の改善率はステロイドの初期量に依存しない。
