腎血管性高血圧症に合併した高血圧性脈絡膜症の1例

要約　19歳女性に頭痛，嘔吐，吐気が突発し，血圧が250/168mmHgに上昇していることが発見された。その2週後に眼科に紹介された。矯正視力は右1.0，左0.05であり，両眼の眼底に網膜深層の白色斑が多発し，乳頭周囲と後極部に漿液性網膜剝離があった。蛍光眼底造影で造影早期から点状過蛍光が多発し，網膜下への色素漏出があり，高血圧性脈絡膜症と診断した。全身検査で血漿レニン活性とアルドステロンの上昇があり，線維筋性異形成による腎血管性高血圧症と診断された。発症から6週後に経皮経管的腎動脈形成術が行われ，血圧は無治療で130/80mmHgに改善した。血圧の改善とともに眼底所見は改善したが，経過中に軟性白斑と線状出血が生じた。加速型高血圧を背景にした脈絡膜循環障害が高血圧性脈絡膜症の原因であると推定された。
