Pit macular症候群に伴う黄斑部漿液性網膜剝離の光干渉断層計および術中所見

要約　目的：Pit-macular症候群により漿液性黄斑浮腫が生じた症例の光干渉断層計（OCT）と術中所見の報告。症例：18歳男性が半年前からの右眼霧視で受診した。矯正視力は右0.9，左1.2であり，右眼黄斑から乳頭に及ぶ網膜剝離があった。乳頭pitが右眼にあった。フルオレセイン蛍光眼底造影でpitは低蛍光で，乳頭耳側に色素貯留があった。黄斑に剝離はなく，網膜内層が分離していた。3か月後に右視力が0.1に低下し，網膜下液が拡大した。OCTで内層の網膜分離と黄斑までに及ぶ網膜剝離があり，pitと網膜分離とが交通していた。硝子体手術とガスタンポナーデを施行した。術中に乳頭内に極小裂孔が発見された。以後，網膜下液は減少し，5か月後に右眼矯正視力が0.3に改善した。結論：本症候群による黄斑剝離では，乳頭小窩内の小裂孔を通って，網膜下液が網膜層間または網膜下に流入する可能性がある。本症例では硝子体手術が網膜の復位に奏効した。
