加齢黄斑変性に対する光線力学療法の視力予後に関与する因子の検討

要約　目的：加齢黄斑変性（AMD）に対する光線力学療法（PDT）の治療成績に関与する因子の検討。対象と方法：AMDに対してPDTを行い，1年以上の経過を観察した76例84眼を対象とした。内訳は男性70眼，女性14眼であり，年齢は51～84歳（平均74歳）であった。内訳は狭義のAMD69眼と，ポリープ状脈絡膜血管症（PCV）15眼である。結果：視力の転帰は，狭義のAMDよりもPCVのほうが良好であった（p＜0.0005）。術後視力は，術前視力と，症状を自覚してから治療までの期間との間に有意な相関があった（各p＜0.0001，p＜0.05）。症状を自覚してから1年以内に治療した症例では術後視力が良好であった（p＜0.0001）。結論：AMDに対するPDTでは，術前視力と術後視力との間に相関がある。発症から1年以内の症例では，術前視力にかかわらず良好な視力改善が得られる。
