白内障手術で高度の遠視化をきたしたフリーフラップ角膜屈折矯正手術後の1例

要約　目的：角膜屈折矯正手術の既往がある眼に白内障手術を行い，術後に高度の遠視が生じた症例の報告。症例：56歳女性が2年前からの視力低下で受診した。8年前にフリーフラップによると思われる屈折矯正手術を受けていた。矯正視力は右0.3，左0.3で，右眼には－3.0Dの近視があった。角膜形状解析で外下方に強く偏心した平坦部が両眼にあった。左眼に白内障手術を行った。ケラトメータでK値は測定不能であったので，ビデオケラトスコープの角膜中央から3mm以内のring 3～12の平均屈折力をK値とし，その値は36.09Dであった。眼軸長は32.12mmであった。SRKⅡ変法で計算した眼内レンズの予測屈折度数は－0.59Dであった。術後＋10.0Dの遠視になったため，予測屈折度が－9.84Dの眼内レンズに交換した。結果，左眼視力は0.8×sph＋1.0Dになった。結論：近視矯正手術で角膜の平坦部が偏心した眼では正確なK値の計算が難しく，眼内レンズの度数決定に注意が必要である。
