Sjögren症候群

Sjögren症候群とは，Henrik Sjögrenによって1933年に報告された臓器特異的自己免疫疾患であり，涙腺や唾液腺をはじめとする外分泌腺を主な標的臓器として，乾性角結膜炎（keratoconjunctivitis scicca）や口腔内乾燥感（xerostomia）を生じる。涙腺，唾液腺以外に障害される外分泌腺として，気道粘膜，胃腸，膵臓，腟の分泌腺，汗腺などがある。Sjögren症候群のみの原発性Sjögren症候群と他の自己免疫疾患を合併する二次性Sjögren症候群に分類され，Sjögren症候群の約50％が二次性といわれる。合併する自己免疫疾患としては，関節リウマチが最も多く（40％，逆に関節リウマチの20％にSjögren症候群を合併），次いで全身性エリテマトーデス（30％），強皮症（20％），多発性筋炎，混合性結合組織病の順に多いとの報告がある1）。また，Sjögren症候群には慢性甲状腺炎（橋本病）や原発性胆汁性肝硬変など，他の臓器特異的自己免疫疾患も合併しうる。
わが国における罹患者数のピークは50歳代，男女比は1：14といわれ，数十万人の患者数が推定されている。原発性Sjögren症候群の約80％（10年以上経過したSjögren症候群の約40％）に全身の臓器病変〔いわゆる腺外症状（外分泌腺以外に現れる病変）〕が合併するとされ，腺外症状として，関節痛，リンパ節腫脹，腎障害，末梢神経炎，Raynaud現象，血液異常，紫斑，紅斑，神経障害，心障害，腎障害，薬剤アレルギーなどがある1）。また，Sjögren症候群には，悪性リンパ腫（Sjögren症候群の5％，通常の約40倍の頻度といわれる1））や原発性マクログロブリン血症の合併が知られる。
