デスメ膜小穿孔を用いた濾過手術の成績

要約　目的：デスメ膜小穿孔を用いた濾過手術の成績の報告。対象と方法：薬物治療で眼圧が22mmHg以上の原発開放隅角緑内障または落屑緑内障65眼を対象とし，デスメ膜小穿孔を用いた濾過手術を行い12か月の経過を観察した。結果：術前の眼圧は平均28.1±5.9mmHgであった。術後3，6，12か月後の眼圧は，それぞれ11.9±2.5mmHg，12.1±2.9mmHg，12.7±3.2mmHgであり，いずれも術前値よりも有意に下降していた（いずれもp＜0.0001）。手術から12か月後の無投薬の状態で，61眼（93％）が20mmHg以下，57眼（86％）が16mmHg以下の眼圧であった。合併症として，低眼圧が2眼（3％），浅前房が2眼（3％），脈絡膜剝離が1眼（2％），前房出血が4眼（6％）にあった。結論：デスメ膜小穿孔を用いた濾過手術で，眼圧が有意に下降した。合併症は低頻度であった。
