眼圧の質のとらえ方

はじめに

 眼圧測定が緑内障診療のキー・プレイヤーであることは論を待たない。正常眼圧緑内障(normal-tension glaucom:以下,NTG)が圧倒的多数を占めるわが国においては,その診断的価値は下がった。しかし,NTGにおいてさえも,眼圧下降が唯一エビデンスの確立した治療法である1)とみなされるようになった昨今,治療効果判定におけるその重要性は減じることはない。

 しかも,緑内障診療時に行われる各種検査法のなかで,眼圧測定はほとんど唯一といってよい他覚的検査法である。すなわち,眼圧測定は何よりも信頼のおける検査法,のはずである。ところが一方で,眼圧測定はさまざまなバイアスがかかる可能性のあることも事実である。医師,患者ともども,1mmHgの変化に一喜一憂するが,はたしてそこにどのような意味が潜在しているのか。本稿では眼圧測定の質のとらえ方について復習してみたい。
