眼瞼からの小刺傷で網脈絡膜まで穿孔して網膜全剝離に至った例の内視鏡硝子体手術での修復と経過

要約　目的：眼瞼経由で眼球への穿孔創による網膜剝離の治療経過の報告。症例：17歳女子の右眼に窓ガラスの破片が刺さり，その翌日に受診した。右眼の矯正視力は0.1であり，球結膜に2mmの創口と網膜全剝離があった。全長が20mmでL字形の強膜創を縫合し，水晶体を温存する硝子体手術を内視鏡を併用して行った。網膜を復位させ，眼内光凝固で裂孔を閉鎖した。瞳孔経由による視認の限界を内視鏡で補うことができた。結果：視力は術後1年で0.9，3年で1.2となった。4年後に白内障が生じ，手術を行った。受傷から7年後の現在，1.2の視力を維持している。結論：穿孔性外傷では，球結膜の創が小さくても強膜が大きく裂けていることがある。本例では迅速な対応が奏効した。
