コンタクトレンズのケア用品による角膜傷害

症例

　患者：37歳，女性

　主訴：コンタクトレンズの定期検査

　現病歴：近視矯正目的のソフトコンタクトレンズ（soft contact lens：以下，SCL）装用中で，現在，自覚症状はない。

　コンタクトレンズ装用歴：従来型のSCLを20年前から使用しており，現在は2週間頻回交換SCLを終日装用していた。現在のケア用品は多目的用剤（multi-purpose solution：以下，MPS）を使用し，ケア方法を遵守していた。

　既往歴・家族歴：特記すべきことはない。アトピー素因なし。

　初診時所見：視力は右（1.2×SCL），左（1.2×SCL）であった。細隙灯顕微鏡検査において，SCLのフィッティングは良好であったが，両眼に軽度の充血がみられた（図1a）。SCLをはずしてフルオレセイン染色を行うと，両眼にA3D2の点状表層角膜症（superficial punctate keratopathy：以下，SPK）を認めた（図1b）。両眼の眼瞼結膜に充血，濾胞や乳頭の形成はみられなかった。涙液検査は，シルマー試験では両眼とも正常で，マイボーム腺の閉塞はみられなかったが，涙液破綻時間は3秒に短縮していた。

　経過：SCLはFDA分類（表1）でグループⅠ（低含水，非イオン性）の素材で，レンズケア用剤は塩酸ポリヘキサニド（以下，PHMB）を消毒成分としたMPSであった。MPSによる角膜障害が疑われたためSCL装用を中止し，抗菌点眼薬の点眼を行った。SCL装用中止後4日目に両眼の充血は軽減し，点状表層角膜症は治癒していた（図1c，d）。以後，SCLの装用を再開に当たって，ポビドンヨード製剤のケア用剤に変更し，現在までSPKの再発はみられていない。
