特発性視神経炎

はじめに

　網膜神経節細胞からの神経線維は，網膜内では無髄神経だが眼球から出て眼窩内に入る際に有髄神経線維となる。約100万本あるとされる神経線維はひとまとめとなって視神経管を通って頭蓋内に入り，視交叉で視神経線維の約半数は交叉し，外側膝状体などに終止する。この眼球から視交叉までの神経線維の束を視神経と呼び，神経線維が集合して眼球を貫く部位は視神経乳頭と呼ばれる。

　視神経に何らかの炎症性の機序が働き，視機能障害をきたした状態を視神経炎と呼ぶ。炎症以外の機序による視神経障害は視神経症と呼ばれる。細菌やウイルスなどの感染が視神経炎の直接の原因となることもあるが，ここでは視神経炎の大多数を占め，脱髄が機序として推測される特発性視神経炎について解説する。また，特発性視神経炎と所見は酷似するが，その病態，予後が異なると考えられる疾患として自己免疫性視神経症（炎）があり，特発性視神経炎の治療を考えるうえで理解が必要となるため，今回一緒に取り上げた。
