はじめに

　黄斑円孔に対し1991年にKellyら^1）により硝子体手術が行われるようになってから，閉鎖率を上げるためにさまざまな治療法が試みられてきた。網膜色素上皮を擦過したり，生体糊を使用したりという工夫がされたが，内境界膜剝離が円孔閉鎖率を格段に向上させることが1995年にBrooks^2）により報告されてからは，内境界膜剝離が一般的な手技になった。

　本稿では内境界膜剝離の手術手技を中心に基本的な術中の注意点についても述べたい。
