糖尿病黄斑浮腫に対するトリアムシノロン後部テノン囊下注入後黄斑部光凝固の長期経過

要約　目的：糖尿病黄斑浮腫に対するテノン囊下ステロイド投与と光凝固による併用治療の長期経過の報告。対象と方法：糖尿病黄斑浮腫17例19眼を対象とした。男性4眼，女性15眼であり，年齢は53～74歳（平均64歳）であった。全例を6か月以上，13眼は13か月まで経過を観察した。トリアムシノロンアセトニド（TCA）20mgをテノン囊下に注入し，その2または4週後に赤色レーザーによる光凝固を疎な格子状に置いた。HbA1C値が7.0mg/dl以下の8眼を血糖コントロール良好，これを超える11眼を不良と定義した。視力と光干渉断層計（OCT）所見から算出した黄斑部網膜体積を指標として効果を判定した。結果：血糖コントロール良好群では治療後の黄斑浮腫が有意に改善持続し，視力にも改善傾向があった。不良群では黄斑浮腫の改善は一過性で，1～3か月間のみであった。結論：糖尿病黄斑浮腫に対するTCAのテノン囊下投与と光凝固による効果は，血糖コントロールが良好な症例で長期間持続した。
