超音波乳化吸引術と眼内レンズ縫着術の同時手術後に強膜炎を起こした1例

要約　目的：超音波乳化吸引術と眼内レンズ縫着術の既往がある眼に強膜炎が発症した症例の報告。症例：糖尿病の既往がある71歳女性が右眼に超音波乳化吸引術と眼内レンズ縫着術を受けた。術後経過は良好で0.7の視力を得た。約2年後から右眼の眼痛，充血，結膜下出血が反復し，強膜炎の診断でステロイドの点眼治療を受けた。病状は軽快したが，再発を繰り返した。その10か月後から強膜炎が増悪しぶどう膜炎が併発した。壊死性強膜炎と診断し，プレドニゾロンのパルス療法を行った。以後，徐々に炎症は鎮静化した。結論：眼内レンズ縫着術後に非感染性壊死性強膜炎が発症することがあり，ステロイドのパルス療法は治療法の選択肢となる。
