免疫再構築症候群によるサイトメガロウイルス網膜炎と肝炎を発症した1例

要約　目的：ヒト免疫不全ウイルス（human immunodeficiency virus：HIV）の感染者に発症したサイトメガロウイルス（CMV）網膜炎と肝炎の報告。症例：30歳代の男性がカリニ肺炎で内科に入院し，HIV抗体とCMV抗原が陽性であることが判明し，精査の目的で眼科を受診した。所見：初診時には眼科的に異常はなかった。その後HIVに対してhighly active antiretroviral therapy（HAART）が開始され，その16日後に右眼に硝子体混濁と，滲出斑を伴う網膜血管炎が生じた。血清トランスアミナーゼが上昇し，CD4陽性細胞が増加していた。ガンシクロビル点滴をバルガンシクロビル内服に変更してから網膜病変と肝機能は改善した。結論：本症例は免疫再構成症候群としてのCMV網膜炎と肝炎であり，バルガンシクロビル内服が有効であった。
