前房注入針を用いた脱臼眼内レンズの眼内縫着術

要約　目的：前房注入針を用いて脱臼した眼内レンズ（IOL）を縫着した報告。対象：対象は，過去に挿入された後房IOLが脱臼した4例と落下した1例である。4例がアクリル製のスリーピース，1例がPMMA製のワンピースIOLであった。方法：まずIOLのハプティックをフックで虹彩上に出し，角膜ポートから27G前房注入針を挿入し，その内腔にハプティックを差し込む。これを眼外に引き出し，ハプティックを角膜ポートから外に出す。これに縫着用針につないだ糸を縫合し前房に戻す。反対側も同様に行う。縫着用針を角膜ポートから再挿入して毛様溝に通糸し，強膜に縫着してIOLを固定する。結果：全5例でIOLを本来の位置に固定できた。併発症は起こっていない。結論：IOLのハプティックが前房注入針に入るときには，本法は脱臼IOLに対する安全で有効な眼内縫着法である。
