加齢黄斑変性に対する光線力学療法後の脈絡膜循環障害を伴う症例に発症した脈絡膜新生血管

要約　目的：光線力学療法後に脈絡膜循環障害が継続し，新たに脈絡膜新生血管（CNV）が増大した症例の報告。症例：77歳男性が6か月前からの左眼視力低下で受診した。矯正視力は右1.0，左0.5であり，minimally classic CNVと診断した。ベルテポルフィンによる光線力学療法を行った。3か月後，照射範囲内に新生血管が出現し，照射範囲に一致する脈絡膜循環障害がある所見が赤外蛍光造影で得られた。初回治療から10か月後の4回目の光線力学療法で，新生血管からの蛍光漏出はなくなったが，矯正視力は0.2に低下した。照射範囲内にポリープ状脈絡膜血管症が生じ，脈絡膜循環障害は継続していた。結論：加齢黄斑変性に対する光線力学療法では，脈絡膜循環障害が生じ新生血管が発症する可能性がある。
