角膜移植後の問題点―創哆開

症例

　患者：16歳，女性

　現病歴：15歳時，左眼の円錐角膜に対し，全層角膜移植を受けた。術後3か月時，犬が顔面に飛びかかり左眼部を受傷，急激な視力低下と流涙を自覚したため当院を受診した。

　受診時の左眼視力は手動弁であった。角膜移植時の縫合糸は9時から3時にかけて半周断裂し，創が哆開して前房は消失（図1），角膜の哆開創に硝子体が陥頓していた。水晶体は透見できなかった。緊急入院のうえ，全身麻酔下で左眼の角膜縫合術，前部硝子体切除術を施行した。術中・術後，感染兆候や網膜剝離などの合併症を認めなかった。

　受傷後6年を経過しているが移植片は透明性を維持し，視力はハードコンタクトレンズによる矯正で（1.5）である（図2）。
