未熟児網膜症の手術治療―新しい流れ

はじめに

　未熟児網膜症の管理は，発症させないための周産期管理から始まっている。未熟児を扱う施設ならば，眼科医は各施設での母体管理から新生児管理の基準，方針を産婦人科医や新生児科医とよく話し合い，眼科診察の基準や時期，治療の適応などの最新情報も含めてコミュニケーションを絶えずとっておくことが必要である。特に，全身状態が悪く多臓器の外科手術を要する児などでは眼科の初診が遅くなりがちであるので，可能な限り基準を逸脱しないよう働きかけ，また，短時間で児に負担をかけない診察技術も磨かなければならない。

　一方，発症した未熟児網膜症の治療の基本は，時期を逸しない適切な光凝固であることは現在も変わりない。しかし，どんなに光凝固を追加しても進行し，さらなる治療を要する重症な未熟児網膜症が存在する。本項では，光凝固で網膜症を鎮静化することができなかった場合の治療について，これまでの手術治療と，最新の試みを紹介したい。
