糖尿病と網膜硝子体―手術

はじめに

　糖尿病に対する啓蒙や内科的治療の進歩，適切な光凝固の施行などにより，以前より重症な増殖糖尿病網膜症は減少したが，それでもなお中途失明の原因の第1位を緑内障と争う状況にあり，視力予後が現在でも不良な疾患といわざるを得ない。

　硝子体手術が適応となるのは増殖糖尿病網膜症（proliferative diabetic retinopathy：PDR）と糖尿病黄斑症であり，PDRと糖尿病黄斑症では治療の目的が異なる。糖尿病黄斑症へは血管透過性亢進による黄斑浮腫に対して視機能の改善を目指した治療であり，また薬物療法などの治療法の選択肢もあって硝子体手術が絶対的な適応とはならない。一方，PDRは網膜虚血から発生した新生血管による増殖性変化に起因した病態であり，硝子体手術は視機能の改善というよりも失明を防ぐことを第1の目的とした治療で，手術以外に選択肢はない。重症なPDRでは増殖膜の処理が難しく，手術の難易度が高いうえに，術中の出血などにより視認性の確保が難しく，それがさらに手術を困難にしている。

　近年，PDRの硝子体手術に大きなブレイクスルーが登場した。ベバシズマブ（アバスチン^®）の硝子体術前投与，照明の進化，23ゲージ（以下，G）や25Gなどの極小切開硝子体手術（micro incision vitreous surgery：MIVS）である。こういった手術補助剤や手術装置，器具の進歩によって，われわれは術中の視認性を向上させて作業効率を向上させ，さらにはより低侵襲な手術が行えるようになった。
