Mooren角膜潰瘍

はじめに

　角膜潰瘍にはさまざまな原因があり，治療方法や予後は大きく異なる。したがって早期に正しく診断し，的確な治療方法を選択することが大切である。一般的に病変部位が角膜中央部に位置するものは感染性であることが多いのに対し，Mooren角膜潰瘍（蚕食性角膜潰瘍）をはじめとする辺縁角膜潰瘍には非感染性のものが多い。

　Mooren角膜潰瘍は角膜輪部に沿って進行する難治性の角膜潰瘍である。穿孔することは比較的少ないものの潰瘍は深く，治癒後も乱視や角膜混濁のために著しい視力障害を生じることがある。その病態は角膜抗原に対する輪部結膜を介した自己免疫反応と考えられている。ステロイド薬による治療を必要とするため，感染性角膜潰瘍を否定することが大切である。
