抗癌剤TS-1^(R)内服による角膜障害の1例

要約　目的：抗癌剤TS-1^(R)を服用中に発症した角膜障害の症例の報告。症例：76歳男性が結腸癌の手術後にTS-1^(R)の服用を開始した。TS-1^(R)にはテガフール，ギメラシル，オテラシルカリウムを含む代謝拮抗薬で抗腫瘍効果があるとされている。1日量100mgを2週経口投与し，2週休薬したのち再び投与する方法をとった。内服開始から11か月後に視力低下を自覚し，その1か月後に受診した。所見：矯正視力は右1.0，左0.8で，右眼には異常がなく，左眼に涙液分泌低下と角膜上皮の凹凸があった。TS-1^(R)の服用を中止し，7か月後に軽快した。左眼角膜の病変に接する輪部には，生検で結膜上皮の角化像があった。結論：本症例はTS-1^(R)に特徴的な角膜障害であると解釈され，これに含まれる薬剤と5-フルオロウラシルが関与している可能性がある。
