抗ウイルス治療に抵抗し両眼網膜剝離を生じた進行性網膜外層壊死の1例

要約　目的：治療に抵抗したヒト免疫不全ウイルス（HIV）感染症に伴う進行性網膜外層壊死（PORN）の1例の報告。症例：47歳男性が3日前からの右眼視野狭窄で受診した。5か月前に急激な体重減少があり，HIV抗体が陽性であった。以後水痘・帯状疱疹，ウイルス性髄膜炎，トキソプラズマ脳症，水痘を発症し，highly active antiretrovirus therapy（HAART）を受けていた。矯正視力は右0.8，左1.2で，右眼には眼底全周に黄白色の病巣と一部に網膜剝離があり，左眼には周辺部に白色の点状病巣があった。前房水から水痘・帯状疱疹ウイルスのDNAが検出され，PORNと診断した。アシクロビルなど抗ウイルス薬の全身投与は奏効せず，初診から16日目に右眼は全網膜剝離になった。3か月後に左眼に黄白色病巣が出現し拡大した。ガンシクロビルなどを投与したが網膜壊死部の円孔から網膜剝離が生じた。硝子体切除術などで網膜は復位した。以後6か月後の現在まで安定し，0.8の視力を維持している。結論：PORNが発症するときには免疫不全状態にあることが多く，抗ウイルス療法や外科的治療が困難なことがあり，予後不良である可能性がある。
