成人発症型卵黄様黄斑変性症と軟性ドルーゼンの鑑別に光干渉断層計が有用であった1例

要約　目的：成人発症型卵黄様黄斑変性症と軟性ドルーゼンの鑑別に光干渉断層計（OCT）が有用であった症例の報告。症例：70歳男性が右眼の変視症で受診した。矯正視力は左右とも1.0であり，右眼黄斑部に1/3乳頭径大の黄色の病変があり，左眼にはこれと似ているがやや崩れたものがあった。眼底所見から成人発症型卵黄様黄斑変性症が疑われた。OCTで病変部の網膜色素上皮の隆起があり，その上の網膜構造は正常であった。成人発症型卵黄様黄斑変性症に特徴的な網膜下の貯留物はなく，他の検査所見と合わせ軟性ドルーゼンと診断した。結論：非侵襲的に網膜の構造や貯留物の有無を描出できるOCTによる検査は，成人発症型卵黄様黄斑変性症と他の疾患との鑑別に有用であった。
