ステロイド関連角膜障害

症例

　［症例1］

　患者：66歳，女性

　主訴：左眼の異物感

　現病歴：10年前からSjögren症候群に対し，クラビット^®点眼および0.1％フルメトロン^®点眼加療が行われていた。3か月前から左眼の異物感を自覚して近医を受診，左眼の角膜潰瘍を指摘された。精査・加療目的で当科を紹介され受診した。

　初診時視力は右0.5（1.2），左0.3（0.6），結膜は軽度充血し，角膜中央部のやや下方に角膜実質の菲薄化した部位（図1a），その中央部に上皮欠損を認めた（図1b）。ドライアイに起因する遷延性角膜上皮欠損と考え，タリビッド^®眼軟膏点入および強制閉瞼により加療したが上皮欠損は拡大した。

　ステロイドによる角膜上皮治癒遅延の可能性を考慮し，0.1％フルメトロン^®点眼の使用を中止した。翌日上皮欠損は消失し，翌々日には左眼は0.5（0.9）まで視力が改善した。
