虹彩角膜内皮症候群

症例

　患者：70歳，男性

　現病歴：約3か月前から右眼の霧視感を自覚して近医を受診したところ，右眼の白内障，左眼の虹彩萎縮と角膜内皮異常を指摘され，精査目的で当科へ紹介された。

　既往歴・家族歴：特記すべきことはない。

　初診時所見：視力は右0.8（1.2×S＋2.00D（）cyl－0.50D 10°），左1.0（1.0×＋1.75D（）cyl－1.00D 10°），眼圧は右15mmHg，左15mmHgであった。細隙灯顕微鏡検査では，左眼の虹彩萎縮と鼻側への瞳孔偏位，角膜内皮面輝度の上昇があった（図1）。

　角膜中央部でのスペキュラーマイクロスコープによる観察では，角膜内皮細胞密度および六角形細胞率の減少と，細胞内にdark areaがあった（図2a）。3時から10時にかけてSchwalbe線を越える周辺虹彩前癒着があった（図2b）。右眼には皮質白内障があったが，角膜や虹彩に異常所見はなかった。

　片眼性に角膜内皮異常，虹彩萎縮，周辺虹彩前癒着があり，眼内炎症はなかったことから，虹彩角膜内皮症候群（iridocorneal endothelial syndrome：ICE syndrome）のサブタイプであるChandler症候群と診断した。

　経過：角膜浮腫による視力低下や眼圧上昇を来していないため，無治療で経過観察中である。
