合剤の国内外への導入状況

はじめに

　欧米で行われた大規模スタディにより，高眼圧症，正常眼圧緑内障，初期から後期の緑内障いずれに対しても，一定の割合あるいは一定のレベル以下に眼圧を下降させることが，視機能の維持に有効な治療法であることが示された。これらのエビデンスに基づき，それぞれの患者で「ベースライン眼圧」を測定し，病型，進行度，年齢やその他の因子を加味して「目標眼圧」を設定することが，日常診療において広く普及するようになった。

　目標眼圧達成のために複数の点眼薬の併用（un-fixed combination）が必要である場合に，どの点眼薬を組み合わせるべきか，点眼する時間帯はどうするべきかについて，24時間の眼圧下降効果，角膜上皮障害などの副作用，点眼回数とコンプライアンスの関係，経済的負担などを考慮した議論がなされるようになった。複数の薬物成分を単一の基剤に混合した「合剤（fixed combination）」の登場は，点眼薬の併用療法に関するいくつかの問題点あるいは患者負担を軽減しうる解決策として注目されている。

　本項では，国内で市販をめざした開発が行われている3種類の合剤を中心に，先行する国外の状況と，合剤の利点と問題点について概説する。
