はじめに

　開放隅角緑内障に対する観血的手術として，表1のようなものが挙げられる。それぞれの術式でどの程度の眼圧下降が得られるのか，その可能性はどのくらいか，そして，術後に何が起こりうるのかをきちんと考えたうえで選択しなければならない。緑内障手術は，よくて現状維持，ある程度の割合で多少見づらさが残り，なかには中心視野を失う場合もある。眼圧がよく下がっても，進行が止まらずに視機能が徐々に失われていく場合もある。もちろん，視機能の低下を防ぐべく最大限の努力を図るべきであるし，そのような工夫を怠るべきではないが，現状では，緑内障手術は緑内障性視神経障害を治す手術ではなく，あくまで眼圧下降術であることを理解すべきである。
