はじめに

　原発閉塞隅角症が生じる機序として，1900年代初頭に概念として想起された相対的瞳孔ブロックが最も一般的である。しかしながら，そのほかにもプラトー虹彩による機序や，水晶体自体による直接的なブロック，あるいは毛様体ブロックなどが複雑に絡み合っていると考えられる。

　従来，急性原発閉塞隅角症（acute primary angle closure：APAC）に対する治療手段としては，薬物療法により鎮静化を図ってのちのレーザー虹彩切開術（laser iridotomy：LI）が広く行われてきたのは周知のごとくである。しかしながら，近年APACに対する初期治療として白内障手術（primary phacoemulsification and intraocular implantation：PPI）^1）が台頭しつつある。

　ここでは，現時点におけるAPAC治療に対するレーザー虹彩切開術の位置づけについて検証してみたい。
