はじめに

　ISGEO（International Society of Geographical and Epidemiological Ophthalmology）分類（別項参照）は，元来疫学調査の比較のための記述方法の統一がその目的であった^1）。その後，この新分類が日本をはじめ各国で臨床分類として使用されるようになってきている^2）。その結果として，レーザー虹彩切開術を中心とした外科的治療の適応は，原発閉塞隅角症および原発閉塞隅角緑内障とされている。急性の原発閉塞隅角症は救急疾患であり，緊急の加療が必要なことは疑いがない。原発閉塞隅角緑内障も瞳孔ブロックを解除する手術療法が必要である。レーザー虹彩切開術が最もよい手術方法であるのか。また，予防という観点から周辺虹彩前癒着もなく，眼圧上昇もない状態である原発閉塞隅角疑い（primary angle closure suspect：PACS）に対してレーザー虹彩切開術を行うのか，結論が出ていないグレーゾーンである。

　本項では，急性および慢性の原発閉塞隅角症に対する治療，および原発閉塞隅角疑いに対する予防治療の必要性を検証しようと行われている中国のスタディの内容について述べる。
