硝子体手術の功罪

はじめに

　近年，米国の多施設前向き臨床研究の成果に基づき，重症な未熟児網膜症に対して厳重な管理がなされてきている。その結果，より早期に十分な網膜光凝固治療を行うことで，多数の症例が網膜剝離に進行することが阻止されている。また，網膜症の活動性が早期に軽減することで，網膜剝離が黄斑部に波及する前（stage 4A期）に硝子体手術を行うことが可能となった。このような早期手術には，欧米で普及しはじめた水晶体温存手術や，わが国でAzumaらが提唱したⅡ型網膜症（aggressive posterior retinopathy of prematurity：AP-ROP）に対する水晶体切除併用手術がある^1,2）。早期手術は，これまで行われてきた硝子体手術と比べてより高いレベルの視機能の獲得が期待される。今後は，いかにして早期に手術を行うかが重要なテーマとなると思われるが，早期手術の歴史は浅く，適応をはじめ不明な点も多い。

　一方，これまで行われてきた硝子体手術は，新生血管の活動性の高い時期を避け，増殖性病変の瘢痕化を待ってから行う手術であり，本稿では「瘢痕期手術」と呼称する（活動期に手術を行うと術中・術後に大量の出血を起こし不成功となる危険性が高い）。瘢痕期手術は長年の手術結果の蓄積があるが，その長期成績については悲観的な結論も多く問題点も多い。しかし，進行した未熟児網膜症に対する最後の治療手段であり，できる限りよい視機能を得られるようにするという課題は残されている。本稿では瘢痕期硝子体手術の治療成績を，今日の視点で再検討しその功罪を論じる。
