劇症型に対する早期硝子体手術

はじめに

　2005年に改定された未熟児網膜症の新国際分類^1）では，従来日本で提唱されていたいわゆる劇症型がaggressive posterior retinopathy of prematurity（以下，AP-ROP）と規定された。その特徴は，眼底の血管の伸びがzone Ⅰやzone Ⅱの後極までと非常に短く，網膜動静脈が全象限で拡張・蛇行し，血管のシャントを形成して，通常のstage 1から順にstage 3に進行するような経過をとらずに急速に網膜剝離に至ることである。

　われわれ眼科医はこれまでこのような重症型に対して，光凝固を行っても進行を止められる例が少なく，しばしば全剝離となり，白色瞳孔となっていく経過を見守るしかなかった。しかしその治療法として，2006年にAzumaら^2）によってAP-ROPに対する早期硝子体手術が紹介され，良好な術後視力が得られる可能性が示唆された。本稿では早期硝子体手術について，抗血管内皮増殖因子（vascular endothelial growth factor：VEGF）療法との併用例なども含めて紹介する。
