抗癌薬（TS-1^®）による角膜上皮障害

症例

　患者：70歳，女性

　主訴：両眼の霧視感

　既往歴：胃癌，癌性腹膜炎

　現病歴：2007年に胃癌に対して手術を受け，同年8月から抗癌薬（TS-1^®，80mg/日）の内服を開始した。投薬のプロトコールは3週間投与，1週間休薬で行われた。TS-1^®内服開始3か月後頃から視力低下を自覚し，同年12月初旬に当科を受診した。

　受診時所見：視力は右0.2（n.c.），左0.15（n.c.），眼圧は右11mmHg，左10mmHgであった。細隙灯顕微鏡検査ではフルオレセイン染色にて，両眼の角膜で上方と下方の角膜輪部から中央に向かう上皮表面の不整と点状表層角膜症様病変を認めた。左眼では角膜中央部付近で偽樹枝状病変も認めた（図1）。

　治療経過：ヒアレイン0.1％点眼液^®とタリビット眼軟膏^®を処方して経過観察を行ったが角膜所見の改善は得られなかった。TS-1^®を処方している外科の主治医と相談のうえ，2008年1月初旬よりTS-1^®をUFT^®に変更した。その後，角膜所見は徐々に改善してほぼ正常となり，視力は右1.0 p（1.0），左0.5（1.0 p）に回復した（図2）。
