水晶体亜脱臼に対する水晶体囊を強膜に縫着固定するフックの試作

要約　目的：水晶体囊を強膜に固定するために試作したフックの報告。対象と方法：試作したフックは，50mmの長さの弱彎針に連結した5-0ポリプロピレン糸からなり，先端は鉤の形で，T字状のパッドを有する。摘出豚眼8個に水晶体亜脱臼をつくり，水晶体乳化吸引術後に眼内レンズを囊内に挿入し，水晶体囊を試作したフックにより毛様溝経由で強膜に縫着した。強膜側の断端は10-0糸で縫合し，強膜フラップで被った。結果：フック部はCCC縁を牽引し，パッド部は水晶体の赤道部を伸展していた。断裂範囲が180°のときには1本，270°のときには3～4本の使用で眼内レンズの中心固定が得られた。結論：試作したフックは水晶体囊を強膜に固定する有用な器具であり，水晶体亜脱臼があっても眼内レンズを囊内に固定できる可能性がある。
