裂孔原性網膜剝離に対する23ゲージ硝子体手術後に生じた細菌性眼内炎の1例

要約　目的：23ゲージ硝子体手術後に細菌性眼内炎が発症した症例の報告。症例：64歳女性が裂孔原性網膜剝離で紹介され受診した。網膜剝離は左眼の上耳側にあり，黄斑には及ばず，裂孔は1つであった。水晶体乳化吸引術と23ゲージ硝子体手術を行い，20％SF₆で硝子体腔を充満し，強膜創は吸収糸で縫合した。手術翌日には問題がなく，2日目に前房蓄膿が生じた。前房と硝子体を洗浄し，眼内に残存するトリアムシノロンを除去した。採取した前房水からEnterococcus faecalisが4日後に培養された。炎症または網膜剝離の再発はなく，手術の17日後に退院した。8か月後の現在，経過は順調である。結論：小切開硝子体手術では眼内炎が続発することがある。その予防として，厳格な結膜の滅菌化や嵌頓硝子体の完全な処理が必要である。
