角膜移植後の問題点―（3）乱視に対するコンタクトレンズ処方

症例

　患者：32歳，男性

　既往歴：喘息

　家族歴：特記すべきことはない。

　現病歴：18年前に両眼の円錐角膜と診断された。15年前（17歳時）に右眼の全層角膜移植術を施行され，術後の経過は良好であった。右眼は視力補正の目的でハードコンタクトレンズを処方されたが，装用感や見え方に満足できなかったため当科を受診した。

　初診時所見：視力は右0.2（矯正不能）（1.0×HCL），左0.15（矯正不能）（1.2×HCL）で，眼圧は右15mmHg，左9mmHg（非接触型眼圧計）であった。細隙灯顕微鏡検査において，右眼の角膜は透明で，角膜上皮障害や角膜後面沈着物などはみられなかった（図1）。角膜内皮細胞密度は右433個/mm²，左2,216個/mm²で，涙液分泌検査（Schirmer試験第1法）は両眼とも1mmであった。角膜形状検査では，右眼のマイヤー像は縦長の楕円形，左眼は角膜中央部から外下方にマイヤー像の不整を認めた（図2）。ハードコンタクトレンズのフィッティングはスティープで涙液交換が不十分であった。

　治療経過：右眼の角膜移植後の角膜形状異常に伴う角膜不正乱視に対し，視力補正のためハードコンタクトレンズを再処方した。再処方したハードコンタクトレンズはlarge and flat法に類似した方法でレンズをフィッティングさせることで，レンズのセンタリングを保ちながら涙液交換が増加し，装用感も改善した（図3）。視力は右（1.2×HCL）であった。現在までに感染などのハードコンタクトレンズによる重篤な眼障害，拒絶反応はみられておらず，内皮細胞密度も500～600個/mm²と維持されている。
