長期間経過し混濁したハイドロジェル眼内レンズの摘出

要約　目的：ハイドロジェル眼内レンズ（IOL）の長期経過と石灰沈着したIOLの摘出法の報告。対象と方法：ハイドロジェルIOLとしてH60Mを2000年に挿入した451眼のうち，315眼について挿入から4年後に石灰沈着の有無を検索した。結果：石灰沈着は236眼（52.3％）にあり，その頻度は挿入からの期間につれ増加した。患者の希望と混濁の程度から設定した摘出条件に従い，78眼（17.3％）でIOLを摘出した。摘出時に合併症としてZinn小帯断裂が12眼（15％）に起こった。前囊を多数切開し，IOLの支持部を回転しない手技を用いてから，その頻度が減った。IOLの再挿入では，後囊切開がされた38眼中31眼（81％）と，していない40眼中16眼（41％）で囊外に固定した。結論：H60M挿入後に石灰沈着が高頻度に発生し，現在なお増加中である。IOL摘出の術式を改良し，摘出時の合併症が減少した。
