はじめに

　現在の白内障手術は，小切開創からの水晶体超音波乳化吸引術（PEA）およびフォルダブル眼内レンズ挿入術の進歩により，従来に比べて良好な術後視機能（quality of vision：QOV）を得ることが可能になった。しかし現在でも，Zinn小帯脆弱例などの眼内レンズ囊内固定が不可能な症例を経験する。水晶体囊内摘出術が白内障手術のスタンダードであった頃は，術後は眼鏡やコンタクトレンズを装用することが当たり前であったが，現在においては良好な術後QOVおよびそれによる良好なQOL（quality of life）を患者は期待している。

　このような症例に対して眼内レンズを挿入する方法は，毛様溝縫着術とその他の方法に大別されるが，本項では毛様溝縫着術以外の方法について述べる。
