薬剤による術中散瞳の意義

　白内障手術において，安全かつ確実に手術を遂行するためには，手術開始時から終了時まで十分な散瞳を維持することが不可欠である。熟練した術者であれば，散瞳状態が悪い症例でも手術は可能かもしれないが，後囊破損や硝子体脱出など術中合併症のリスクが高くなるため^1），可能な限り十分な散瞳を確保するべきである。

　一般的には，手術時に良好な散瞳を得るため，塩酸シクロペントラート，塩酸フェニレフリン，トロピカミドなどの散瞳薬や，散瞳維持効果のある非ステロイド系消炎点眼薬など，複数の点眼液を術前から頻回点眼する。しかしながら，偽落屑症候群^2,3）や糖尿病症例^4）など術前点眼のみでは十分な散瞳が得られない症例や，手術開始時には散瞳良好であったにもかかわらず，術中早期に縮瞳してしまう症例をしばしば経験する。また，小児や循環系疾患のある患者では，散瞳薬が全身へ影響する可能性がある^5～7），前房が浅い症例では，術前散瞳による急性緑内障発作を引き起こす可能性があるなどの理由から，散瞳薬の頻回点眼を避けたい症例もある。そのような散瞳不良症例や散瞳薬の頻回点眼を避けたい症例に対しては，術中に散瞳させるというアプローチが必要になってくる。
