線維柱帯切開術およびその近縁手術の基本手技とその術後管理

ここが変わった！

以前の常識

●線維柱帯切開術（トラベクロトミー）は，金属プローブ（トラベクロトーム）を用いて切開する。

●トラベクロトームを使った切開範囲は最大120°までである。

●トラベクロトームを回転させて切開するときには抵抗があり，正しく回転させなければDescemet膜剝離を起こす危険もある。周辺虹彩前癒着があると，更にリスキーとなる。

●トラベクロトミーを行うには，ある程度緑内障手術に習熟する必要がある。

●さらなる眼圧下降を求めるなら，サイヌソトミー（濾過手術の一種）や深層強膜弁切除術などを併用したハイブリッド手術を行う。

●ぶどう膜炎などによる続発緑内障にはあまり有効ではない。

現在の常識

●線維柱帯切開はトラベクトーム（trabectome^®）や糸など金属プローブ以外のツールによっても切開できるようになった。

●糸を用いる変法は，360°全周切開はもちろん，180°切開など任意の範囲を切開することが可能である。

●トラベクトーム^®やアイ・ステント（iStent^®）の登場により，緑内障手術の経験がない術者でも，トラベクロトミーと同様な手術を行うことができるようになった。

●糸を用いた変法では，切開時にまったく抵抗を感じることなく安全に切開でき，たとえ周辺虹彩前癒着があっても可能である。

●全周切開する360°トラベクロトミー変法では，ハイブリッド手術を併用しなくても高い眼圧下降効果が得られ，ぶどう膜炎による続発開放隅角緑内障にも有効である。
