ここが変わった！

以前の常識

●高血圧や細動脈硬化に伴い網膜血管には変化がみられ，眼底検査は循環器検診に用いられている。

●循環器検診における眼底検査は主観に基づく判定が主であり，再現性に難がある。

●さらに重症高血圧患者が減少した現在では，循環器検診における眼底検査の占める臨床的意義は低くなった。

現在の常識

●定量的に網膜血管の変化を評価する方法が確立されつつあり，より微細な変化をとらえることが可能となった。

●その結果，高血圧の有病を示すだけでなく，高血圧の発症予測や治療効果判定にも応用が可能となった。

●さらに脳卒中などの循環器疾患の発症に，眼底の変化が高血圧とは独立して関連することがわかってきた。

●網膜血管には高血圧や細動脈硬化だけでなく，糖尿病，メタボリックシンドロームや肥満，慢性腎臓病，炎症性疾患などさまざまな疾患との関連が示されるようになり，今後も全身疾患の血管変化を評価するうえで重要な指標となりうる。
