甲状腺眼症における眼窩減圧術の適応

ここが変わった！

以前の常識

●眼窩下壁や内側壁をターゲットとした眼窩減圧術が施行されてきたが，術後複視を高率に合併した。

●上記合併症のため，眼窩減圧術の適応は圧迫性視神経症や眼球突出に伴う角膜障害に限定されていた。

現在の常識

●術後複視の発生が非常に少ない外側深部眼窩減圧術や眼窩脂肪除去による減圧術が報告されて以降，眼窩減圧術の適応は眼球突出による醜形や眼窩内うっ血に伴う諸症状にも拡大した。

●外側深部眼窩減圧術単独または内側眼窩減圧術との併施，さらに眼窩脂肪除去を追加することによって，最小限の合併症で効果的な減圧が可能となった。
