硝子体手術―小切開硝子体手術

はじめに

 25ゲージの手術器具を用いて経結膜的に行う硝子体手術システムが2002年にJuanらによって発表され,現在の小切開硝子体手術(micro-incision vitrectomy surgery:以下,MIVS)の始まりとされている1,2)。硝子体手術の歴史を振り返ってみると,かつてオープンスカイ手術であった硝子体手術が1971年にMachemerによって初めてclosed eye surgeryとして確立され3),その後さらにO'Malleyら4)によって現在のような20ゲージの3ポートシステムへと発展したが,硝子体手術は約30年の年月を経てこのMIVSの登場によってようやく新しいステージに進んだ感がある。

 黎明期にはいろいろと問題点の多かったMIVSも,ここ数年の手術手技の進歩,手術器具の改良,新しい照明系や観察系の開発,そして合併症の発症機序の解明などMIVSに対する多くの臨床研究の積み重ねを通して,この新しい術式に対する理解が深まった。MIVSは硝子体手術の新しい領域としてその地位を確立しつつ,いまもなお進化し続けている。本項ではMIVSの現状をサマリーし,今後の開発を含めた将来的な展望について紹介する。
