IOLマスターを用いた線維柱帯切除術後の眼軸長測定

要約　目的：線維柱帯切除術の前後に眼軸長を測定した結果の報告。対象と方法：過去1年間に線維柱帯切除術を行った77眼を対象とした。27例30眼では単独手術，40例47眼では白内障手術を同時に行った。平均年齢は61.7±10.8歳であった。眼軸長はIOLマスターで測定し，眼内レンズの度数はSRK-T式を使用し，屈折は自動屈折計で測定した。結果：平均眼圧は術前19.3±5.4mmHg，術後11.0±4.2mmHgであった。術後平均140日後の眼軸長は術前よりも有意に減少した（p＜0.0001）。眼軸長の変化は，眼圧の変化（p＜0.0001）と，年齢（p＝0.0003）との間に有意な相関があった。線維柱帯切除術の単独手術では，屈折値に有意な変化がなかった。同時手術群では，実測屈折値が予想値よりも近視化した。結論：線維柱帯切除術と白内障同時手術では，術後に近視寄りの屈折誤差が生じる可能性がある。
