視神経乳頭の診かた

はじめに

　視神経乳頭を観察するという行為は，眼科医が非常に多くの頻度で行うことのひとつである。乳頭の色，形状，大きさ，陥凹や辺縁の状態，乳頭上や周囲の神経線維や血管走行など，種々の要素を瞬時に見分けることが，多忙な日常診療では要求されている。しかし，乳頭はバリエーションが多く，いかに訓練を受け，専門家と称される眼科医でも，上記の各要素を，症例によらず瞬時に判断することは不可能に近いことである。

　「観察」とはただ見ることではなく，文字通り考えながら観ることであり，問題意識を持って，要素ごとに評価をすることが重要である。
