視野で何がわかるか，視神経疾患

はじめに

　CTやMRI画像で，視路の病変が明確に映し出される以前には，対座法，黒板法，Foerster視野計，Goldmann視野計などを利用した視野診断が，視路の局在診断として極めて重要な位置にあった。以前の眼科医は同名半盲，両耳側半盲とそのバリエーションを読む力量を問われたものだ。

　一方，視神経疾患あるいは視神経症といえば，視力低下があり，視野では図1に示すような中心暗点，盲斑中心暗点が出現するというのが相場である。そうした中で，病巣局在診断としての視野の意義はやや低下し，定性視野から定量視野が主流になった今日では，臨床医の関心は疾患ごとに視野に何らかの特異性はないだろうか，そしてそれが臨床診断の根拠のひとつとして使えないだろうかということに移り，それに対応する研究が少なからず行われるようになった。

　本稿ではそうしたものを取り上げながら，視野を用いた特異的臨床診断がどこまで可能かを考えてみたい。
