眼窩内疾患の診かたと治療―慢性炎症

はじめに

　眼窩の炎症でも，身体の他の部位と同じく炎症の3徴である発赤，腫脹，疼痛がみられる。しかし，眼窩炎症では感染性の眼窩蜂巣炎のような炎症の強いタイプから偽腫瘍を形成する腫瘤性病変の強いタイプまでさまざまなタイプがあり，慢性炎症では腫瘤性病変が強く現れることが多く，発赤はそれほど目立たないが，稀に非常に強い疼痛を訴えることがある。慢性眼窩炎症で最も頻度が高いものは甲状腺眼症であるが，それ以外にもさまざまなものがある（表1）^1,2）。最近では自己免疫性のIgG4関連眼窩症^3,4）やビスホスホネートなど骨粗鬆症予防薬投与に伴うものが注目されている^5,6）。本シリーズではすでに甲状腺眼症を取り上げているので，本稿では主として特発性眼窩炎症，特発性外眼筋炎，IgG4関連眼窩症などの甲状腺眼症以外の疾患について述べる。
