Ex-PRESSTM（エクスプレス）挿入術

線維柱帯切除術（trabeculectomy：TE）は代謝拮抗薬（マイトマイシンCなど）の併用によって最も大きな眼圧下降作用が期待できる術式であり，現在の緑内障濾過手術として標準的術式となっているが，一方で術後早期合併症（低眼圧，浅前房，脈絡膜剝離，前房出血など）の頻度が高く，問題の多い術式でもある。その術後早期合併症を減少させる目的で，1999年以降，海外で臨床使用されてきたEx-PRESSTM（エクスプレス）が2011年12月，日本でも医療器具として認可された。さらに2012年5月，このデバイスを用いた「緑内障治療用インプラント挿入術」が保険適応されることになり，ようやく日本における一般的な臨床使用が可能になった。エクスプレス（日本での正式販売名：アルコンエクスプレスTM緑内障フィルトレーションデバイス）は，インプラントの1種であるが，緑内障手術用ミニデバイスとも呼ばれ，海外ではすでに汎用されている。
筆者は日本で認可を受ける以前の2008年2月より，大学倫理委員会の承認のもと，informed consentの得られた患者を対象に，個人輸入したエクスプレスを使用してきた1,2）。その経験も踏まえて，エクスプレスを用いた手術の概要や成績について従来の報告をまとめ，私見を述べてみたい。
