はじめに
緑内障手術で現在最も広く行われているトラべクレクトミー（trabeculectomy）は，眼圧下降効果，汎用性において最も優れた術式である。しかし，その眼圧下降効果は非生理的房水流出路である濾過胞に依存するため，術後早期の低眼圧に伴う合併症や，後期に起こる眼内炎などの重篤な併発症が問題となっている。
カナロプラスティ（canaloplasty）は2007年にLewisら1）によって報告された濾過胞をもたない非穿孔性緑内障手術の術式の一つである。深層強膜弁切除後，Schlemm管を粘弾性物質にて拡張し（viscoanalostomy），さらに10-0プロリン糸をSchlemm管内に360°通糸し縫縮することにより房水流出路の主経路である線維柱帯流出路を前房へ突出・伸展させ，流出抵抗の低下を図る術式である（図1，2）。欧米の報告では術後眼圧は15mmHg前後と，トラべクレクトミーには劣るもののトラベクロトミー（trabeculotomy）と同等以上の成績を示している。濾過胞に依存しない生理的房水流出路再建術であるため，濾過胞に伴う合併症や術後のケアの必要がない。また眼内に穿孔しないため術中の極端な眼圧低下もなく，トラべクロトミーなどにみられる大量の前房出血やそれに伴う眼圧上昇といった合併症の頻度も低いのが利点である。
