Sjögren症候群

POINT

◎Sjögren症候群は涙腺，唾液腺のリンパ球浸潤を主体とする慢性の自己免疫疾患で，多彩な自己抗体が出現する。その発症には遺伝的要因，ウイルス感染などの環境要因，免疫要因，加齢などが複雑に関与する。

◎眼乾燥，口腔乾燥を主徴とし，中年女性に好発する疾患であり，膠原病に合併する続発性Sjögren症候群と合併しない原発性Sjögren症候群に分類される。原発性Sjögren症候群では涙腺，唾液腺以外の全身多臓器の病変（腺外病変）が半数にみられ，一部の症例では悪性リンパ腫を発症する。

◎内科，歯科・口腔外科と連携して診断・治療にあたることが望ましい。口腔乾燥にはムスカリンM3受容体作用薬の全身投与が適応となる。

◎Sjögren症候群における眼乾燥の治療は通常のドライアイと同様であるが，Sjögren症候群ではより重症である場合が多く，障害が高度であれば積極的に涙点閉鎖を治療の選択肢とする。
